First you will need to work with the family to establish whether they are eligible for the service by completing the following Camden Futures Eligibility Checklist.
If the family are eligible for the service you can make a referral. The method of referral will depend on whether your organisation is registered on the Camden eCAF system.
If your organisation is registered on the eCAF system the quickest and easiest way of making a referral is to to complete an eCAF assessment and send it to Camden Futures Co-ordinator on the eCAF system. This will go straight to the Camden Futures Co-ordinator Francesca Cappelli who is based at Elfrida Rathbone Camden.
If you are a London Borough of Camden Social Worker you may make a referral using a completed initial assessment or core assessment.

If your organization is not registered on the eCAF system you will need to complete a Camden Futures Online Referral Form or download the Camden Futures Referral Form into Microsoft Word and email the completed form to firstname.lastname@example.org or fax it 020 7424 1605.

Camden Futures Referral Network
A referral network has been developed of local agencies with links to the Camden Futures services. These agencies will be able to easily make referrals to or receive referrals from Camden Futures.
